Cardiology Referral Form

Dear Colleague,

We are excited to announce the opening of Capital Cardiology's second location, Capital Cardiology West, in the West End of Ottawa. This new facility is conveniently located in Kanata, just off the Terry Fox exit on Highway 417, and offers ample free parking. Along with myself, our team of Cardiologists and Cardiovascular specialists will be available to provide cardiology consultations alongside an array of cardiac diagnostic services.

Our services include:

● Echocardiogram  

● Stress Echocardiogram (Treadmill)  

● Ambulatory ECG Monitoring (Holter)  

● Ambulatory Blood Pressure Monitoring  

● Myocardial Perfusion Imaging (conducted at our central location).

To simplify the referral process, we have enclosed a medical requisition form for your convenience. We welcome cardiology referrals and will begin accepting new ones on July 15, 2023, via fax at (613) 270-0553. Additionally, we are pleased to offer electronic referrals for a more streamlined experience.
