Referral Form

For Accuro EMR users

Follow the steps below for "Form " Installation Instructions:

Open Accuro and select the Accuro “Bullseye” (red icon at bottom-left of the screen).
Click “Tools” then “Form Editor”. A new box will pop-up.
Click ”Tools” again then “Publish/Download”.
Ensure category is in “Standard Forms”.
Search for your applicable requisition form e.g. Capital Cardiology – Ottawa- Cardiac Test. The Publisher should be “Capital Cardiology”.
Click “Preview” as needed then click “Download” to complete.

Dear Colleague,

It is our great pleasure to announce the opening of Capital Cardiology's second location

(Capital Cardiology West) in the West End of Ottawa, situated in Kanata, conveniently

located just off the Terry Fox exit on Highway 417, with ample free parking available. Along with

myself, our team of Cardiologists and Cardiovascular specialists will be providing Cardiology

Consultation in conjunction with a range of cardiac diagnostic services.

Our services include:

● Echocardiogram

● Stress Echocardiogram (Treadmill)

● Ambulatory ECG Monitoring (Holter)

● Ambulatory Blood Pressure Monitoring,

● Myocardial Perfusion Imaging (to be performed at our central location).

To facilitate the referral process, please find enclosed a referral form.

We will be accepting new referrals beginning July 15 , 2023, via fax at (613) 270-0553.
